We read with interest the paper on massive blood transfusion
\n(MBT) and outcome in polytrauma patients in Germany
\npublished by Huber-Wagner et al. in a recent issue of Vox
\nSanguinis [1], and would like to comment on the incidence
\nof MBT at our institution (a 700-bed university hospital). We
\nsearched blood bank records for MBT episodes occurred from
\nJanuary 2001 to December 2005, and patient\u2019s clinical data
\nwere exclusively gathered from the blood requesting form.
\nOverall, 304 episodes of MBT were identified in 288 patients
\n(upper digestive bleeding, elective surgeries, cardiovascular
\nsurgeries, emergency surgeries, or polytrauma). Mortality
\nrate was 48%, and multivariate analysis identified age [odds
\nratio (OR) 1\u00b7023, 95% confidence interval (CI) 1\u00b7006\u20131\u00b7040]
\nand number of red blood cell (RBC) units transfused within
\nthe first 24 h (OR 1\u00b7094, 95%CI 1\u00b70032\u20131\u00b7160) as weak but
\nsignificant independent predictors for mortality, whereas
\npolytrauma diagnosis was associated with the lowest mortality
\n(OR 0\u00b7325, 95%CI 0\u00b7112\u20130\u00b7940).<\/div>\n","protected":false},"excerpt":{"rendered":"
