Preoperative anemia is an important risk factor
\nfor perioperative red blood cell transfusions and has been
\nshown to be independently associated with adverse outcomes
\nafter noncardiac surgery. The objective of this observational
\nstudy was to measure the prevalence of preoperative anemia
\nand assess the relationship between preoperative anemia and
\npostoperative mortality.
\nMethods: Data were retrospectively collected on 7,759 consecutive
\nnoncardiac surgical patients at the University Health Network
\nbetween 2003 and 2006. Preoperative anemia was defined as
\na hemoglobin concentration less than 12.0 g\/dl for women and
\nless than 13.0 g\/dl for men. The unadjusted and adjusted relationship
\nbetween preoperative anemia and mortality was assessed
\nusing logistic regression and propensity analyses.
\nResults: Preoperative anemia was common and equal between
\ngenders (39.5% for men and 39.9% for women) and was
\nassociated with a nearly five-fold increase in the odds of postoperative
\nmortality. After adjustment for major confounders
\nusing logistic regression, anemia was still associated with increased
\nmortality (odds ratio, 2.36; 95% confidence interval,
\n1.57\u20133.41). This relationship was unchanged after elimination
\nof patients with severe anemia and patients who received transfusions.
\nIn a propensity-matched cohort of patients, anemia
\nwas associated with increased mortality (odds ratio, 2.29; 95%
\nconfidence interval, 1.45\u20133.63).
\nConclusions: Anemia is a common condition in surgical patients
\nand is independently associated with increased mortality.
\nAlthough anemia increases mortality independent of transfusion,
\nit is associated with increased requirement for transfusion,
\nwhich is also associated with increased mortality. Treatment
\nof preoperative anemia should be the focus of
\ninvestigations for the reduction of perioperative risk.<\/div>\n","protected":false},"excerpt":{"rendered":"
