The objective of this observational<br \/>\nstudy was to measure the prevalence of preoperative anemia<br \/>\nand assess the relationship between preoperative anemia and<br \/>\npostoperative mortality.<br \/>\nMethods: Data were retrospectively collected on 7,759 consecutive<br \/>\nnoncardiac surgical patients at the University Health Network<br \/>\nbetween 2003 and 2006. Preoperative anemia was defined as<br \/>\na hemoglobin concentration less than 12.0 g\/dl for women and<br \/>\nless than 13.0 g\/dl for men. The unadjusted and adjusted relationship<br \/>\nbetween preoperative anemia and mortality was assessed<br \/>\nusing logistic regression and propensity analyses.<br \/>\nResults: Preoperative anemia was common and equal between<br \/>\ngenders (39.5% for men and 39.9% for women) and was<br \/>\nassociated with a nearly five-fold increase in the odds of postoperative<br \/>\nmortality. After adjustment for major confounders<br \/>\nusing logistic regression, anemia was still associated with increased<br \/>\nmortality (odds ratio, 2.36; 95% confidence interval,<br \/>\n1.57\u20133.41). This relationship was unchanged after elimination<br \/>\nof patients with severe anemia and patients who received transfusions.<br \/>\nIn a propensity-matched cohort of patients, anemia<br \/>\nwas associated with increased mortality (odds ratio, 2.29; 95%<br \/>\nconfidence interval, 1.45\u20133.63).<br \/>\nConclusions: Anemia is a common condition in surgical patients<br \/>\nand is independently associated with increased mortality.<br \/>\nAlthough anemia increases mortality independent of transfusion,<br \/>\nit is associated with increased requirement for transfusion,<br \/>\nwhich is also associated with increased mortality.
