Abstract Purpose: This study
\nwas undertaken to investigate the
\nefficacy of red blood cell transfusion
\n(RBCT) at reversing the deleterious
\neffects of moderate anemia in critically
\nill, non-bleeding patients.
\nMethods: This was a retrospective,
\npair-matched (ratio 1:1) cohort study.
\nNon-bleeding critically ill patients
\nwith moderate anemia (nadir hemoglobin
\nlevel between 70 and 95 g\/l),
\nadmitted to the ICU over a 27-month
\nperiod, were included. Anemic
\npatients were included upon meeting
\nfive matching criteria of having the
\nsame nadir hemoglobin (\u00b15 g\/l),
\nAPACHE II score (\u00b15), SOFA score
\n(\u00b12), admission diagnostic group,
\nand age (\u00b15 years). Outcome events
\noccurring over the whole ICU stay
\nand after RBCT were collected. After
\nhospital discharge, all patients had a
\n2-year follow-up period. Results:
\nTwo hundred fourteen non-transfused
\nanemic patients (NTAPs) were successfully
\nmatched with 214
\ntransfused anemic patients (TAPs). In
\naddition to the matching criteria, at
\nbaseline, both groups were homogenous
\nwith respect to multiple
\ncomorbidities. Compared with TAPs,
\nNTAPs showed significantly lower
\nrates of hospital mortality (21
\nvs.13 %, respectively; p\ufffd.05) and
\nICU re-admission (7.4 vs. 1.9 %,
\nrespectively; p\ufffd.05). Additionally,
\nNTAPs had significantly lower rates
\nof nosocomial infection (12.9 vs.
\n6.7 %, respectively; p\ufffd.05) and
\nacute kidney injury (24.8 vs. 16.7 %,
\nrespectively; p\ufffd.05). Similar
\nresults were obtained in subgroup
\nanalysis where only more anemic
\npatients (68 matched pairs) or patients
\nwith cardiovascular comorbidities (63
\nmatched pairs) were considered.
\nConclusions: RBCT does not
\nimprove the clinical outcome in nonbleeding
\ncritically ill patients with
