It is usually due<br \/>\nto combination of various mechanisms, including: pre-existing<br \/>\nanaemia prior to surgery; anaemia due to impaired erythropoiesis,<br \/>\nincluding alterations of metabolism of iron and erythropoietin<br \/>\n(EPO); anaemia due to increased destruction of red blood cells<br \/>\n(RBCs); and anaemia due to iatrogenic causes. Postoperatively,<br \/>\nanaemia resembles anaemia of chronic disease and is probably<br \/>\nrelated to the effects of inflammatory mediators released during<br \/>\nand after surgery on the production and survival of RBCs. Proinflammatory<br \/>\ncytokines, such as tumour necrosis factor, impair<br \/>\nerythropoietin-dependent signalling and iron homeostasis. Iatrogenic<br \/>\ncauses, notably excessive phlebotomies, remain a major<br \/>\ncause of perioperative anaemia. With increasing emphasis on<br \/>\nrestrictive blood transfusion strategies, understanding these<br \/>\nmechanisms is important for the clinician.<\/div>\n","protected":false},"excerpt":{"rendered":"<p>Editorial: Best Practice Fecha: 01\/12\/2012 Sumit Singh, MD, , Vadim Gudzenko, MD, Mitchell P.
